The patient register of Kipekee
4. THE PURPOSE OF HANDLING PERSONAL INFORMATION
Examinating, planning, executing and following up the treatment of patiens and customers. Planning and recording the registars own activities. With a written permission from the customer the contact information can be used for marketing and informative purposes.
5. THE CONTENT OF THE REGISTER
Name of the patient/customer, date of birth and contact information.
The necessary preliminary information and health status of the patient/customer.
Other necessary information regarding the treatment.
Information about bookkeeping and invoicing.
6. THE SOURCES OF THE REGISTER
The patient/customer self and the guardian of underage children. The information that has occured during examination and treatment. Documents received from other treatment- or rehabilitation units with the consent of the patient/customer and the guardian of an underage child.
7. GIVING OUT INFORMATION FROM THE REGISTER
Personally to the patient/customer, unless there is a legal obstacle to do so. With a special legal order to authorities, research units and insurance companies.
8. TRANSFERRING DATA OUTSIDE OF EU AND ETA
The register does not give out infromation outside of EU or ETA.
9. THE PRINCIPLES OF KEEPING THE REGISTER PROTECTED
Manual documents are kept in a locked closet in a locked and supervised space. Electronic documents are kept in a folder protected with password behind the user account of the contact person of the registar. The personal information is recorded based on the written approval of the patient/customer. Only the person treating the patient/customer has the right to read and make records to the regiser of the patient.
10. THE RIGHT TO CHECK THE REGISTAR
The patient/customer has the right to check their own records. A written request for seeing records should be delivered to the registar. The information is given in writing. Before giving out any information the patient/customer must provide a proof of identity with an image.
11. THE RIGHT TO REQUEST A CORRECTION
The patient/customer has the right to request correction false information in the patient record without delay. The request must be presented in writing and delviered to the contact person of the registar.
